Our Complete Care Management® process will help you identify the right care for your unique situation and keeps track of your care even when we’re not there.
Step 1: Provide education on your initial call that will help you think about the options that may be available to you.
Step 2: Provide a no cost, no obligation, in home assessment to the person in need of care by one of our Nurse supervisors to determine the appropriate care for their situation. We may also recommend other service providers if we believe that a different approach to care is in the person’s best interest.

Step 3: If appropriate, our Nurse Supervisor will develop a plan of care and we will identify a Caregiver that has the necessary training, experience and skills for the person needing care. We will also identify someone that should be a good personality match for the person needing care. Please see our List of Services that can be provided by our Caregiver’s.
Step 4: Work with the Medical Team of the person needing care to understand their plan of care and coordinate the execution of the plan of care with or for the family.

Step 5: Provide Nurse re-evaluations as needed but at a minimum every 60 days. We will also do nurse check-ins every 30 days. We may also do occasional visits to ensure that you are satisfied with the service you are receiving from our Caregiver and our Company.
Step 6: Making Your Life Easier. We believe our job is to make your life better and easier. We often get special requests for assistance needs that are outside of the service we normally provide. If we can ever be of assistance to any of our clients, we will do our best to accommodate their needs.
