24 Hour Home Care

24 Hour Home Care

24 Hour Home Care involves round the clock service for clients. Home Care Agencies charge an hourly rate for this service. Caregiver staffing for 24 hour Home care usually involves multiple caregivers working in shifts based on the client’s needs, preferences and overtime pay implications. There can be breaks for the caregiver depending on the client needs and the plan of care requirements. However, the caregivers do not typically reside at the client’s home.

Live In Home Care

With our live-in home care services, one of our certified caregivers will live with your loved ones in their home.  Our live-in caregiver will stay in their own space within the home and live there for 3-4 days at a time, alternating time with a second caregiver.  While in the home, our caregiver will tend to the needs of your loved ones and help them through their daily routines.  You and your loved ones will have peace of mind knowing that our live-in caregiver is always there to respond to emergencies and provide good companionship.

Our live-in caregiver will start first thing in the morning by helping your loved ones get out of bed and through their morning routine, including washing up, getting dressed, and preparing breakfast.  Throughout the day, our caregiver will be there to help with medication, light housekeeping, and transportation if your loved one needs to run errands or make it to a doctor appointment.  Live-in home care is ideal for seniors who have recently returned home from the hospital or surgery, those who have lost their spouse, and those who want an all-day companion.

Your Individualized Plan of Care:

Every client receives an individualized Plan of Care. It’s our way to ensure you receive the service you need and everyone is prepared for the care that needs to be delivered.

  • Step 1: Certified Nurse Assessment: Our Certified Nurse and Care Coordinator visits each client and/or family member at their place of residence to evaluate condition, discuss options, and develop a Plan of Care.
  • Step 2: Homecare Assistance Benefit Determination: We help identify any programs, community resources, or benefits that might be available for our clients such as: Medicare/Medicaid Services, Private Health Insurance, Long Term Care Insurance, VA pensions and others.
  • Step 3: Plan of Care: We incorporate all the orders and recommendations from your physicians, case managers, healthcare providers and our own initial assessment in order to develop an effective Plan of Care. This plan will describe all the services required, the proposed frequency, cost of each service, potential equipment and supplies needed and the possible payment sources.
  • Step 4: Caregiver Matching and Assignment: Every time a new caregiver is assigned to a client, he or she receives a copy of the plan of care and discusses it with a nurse or case manager. In some complex or delicate cases (such as advanced dementia, severe mobility impairments, autism or post-surgical care), the caregivers undergo complementary training provided by our nurses, which refreshes important case-specific techniques or care aspects that must be observed.
  • Step 5: Ongoing Management and Check-Ins: Plans of care are continuously updated, as a consequence of supervisory visits, experience accumulated on the case by our caregivers, and interactions with the client, healthcare professionals or family members. Every update is shared with all caregivers assigned to a client, and case managers follow up with individual aides to ensure adherence to the updates.

What our client say about us

“The staff was most helpful and considerate of all involved. They treated us with the utmost respect and gave the best care possible. They always went out of their way to assist in anything that was needed. If it weren’t for assisting hands the process would have been impossible. We highly recommend them to any family considering working with them.”
-Steve. L

“The team at Assisting Hands has been a huge help for my mom. Mom has been happy with all of the caregivers they have provided, and she says they are all very attentive and caring. I have appreciated the excellent communication with Assisting Hands, and also their flexibility and reliability, as well as — of course — their great care for my mom.”
– David G.  – 5 stars

“On October 1,2016 my Mom and best friend passed away. For the past 3 years she lived with my husband and me. A friend recommended Assisted Hands to help me. Without their devoted staff, and caring helpers my experience would have been a lot harder. They treated my Mom as if she were their Mom. I can’t thank the entire staff enough for all they have done. You are an amazing group of people.”
